First, excellent form. If you're going to get hit by a car, damaging the car and getting away without broken bones is the way to do it. And kudos to the shots of the damage to the car and the hospital pic. You also scored big with sympathetic police.
I strongly suggest having Ilya take a good look at the bike. It might seem ok at first blush, but a good inspection is essential. Especially since the criminal should pick up the tab.

I also got hit by a car on Sunday, but the Madone and I were very lucky this time. A car decided to drive into the bike lane while I was next to them on Bergen. I slammed on my breaks and skidded into the side of the car in front of the driver, but I stayed upright and on my bike.
I had the police come and write up a report just in case.
If anything it's a reminder to go slow and pay more attention while riding in the streets.
@mugwump thanks for the reminder to get the bike checked. I think the fork at a minimum needs to get replaced.

Quick follow up on my post / takeaways from my
meeting w/ L&C.
1) get police report
and go to the hospital. You can get collision/vehicle related police
reports from any precinct. It costs $10 but I didn’t get charged. The “Ins Code”
box on the police report denotes the motorist’s insurance provider.
2) witnesses—get their information before they leave the scene or keep them around until the cops arrive.
3) No fault. Do
not use your medical insurance for anything related to the accident. Process it
as no fault. You may have to call the hospital and other medical providers at a
later point, after you get a claim number, which takes a few days. Basically by
law your medical insurance provider does NOT have to pay your medical bills if you
were hit by a motorist and you are not at fault. If you use your medical
insurance and the insurance company connects the dots, they would kick the bill
back to you and not cover anything. This is important because there is a
limited amount of time (45 day window) for which you have to file for no fault
and submit a claim. If you present your medical insurance at the hospital and
it gets kicked back outside of that 30/45 day no fault window, you are screwed.
You'll have a medical bill that needs to get paid out of your pocket.
4) Your auto insurance
and umbrella coverage. I never appreciated the usefulness of the Uninsured
Motorist & Underinsured Motorist component of an insurance policy until I
got hit by someone driving a crappy car. Basically NY state requires motorist
to have at least $25k of coverage and then there are hit and runs. So to the
extent there is property damage, medical expenses, pain and suffering, loss of
income, etc. that exceeds the motorist's coverage (or lack thereof) / policy
limit, the “Uninsured Motorist & Underinsured Motorist” component of your insurance
policy comes into play. For example, the guy that hit me was driving a 2006
Honda Civic and has $25k of coverage. I have a car and an insurance policy that
allows for up to $100k of Uninsured/Underinsured Motorist coverage. If my total
damages exceed $125k, I am probably SOL. Phil recommends we all max out
Uninsured/Underinsured Motorist coverage (my policy allows for up to $500k and
only costs me an additional $20/year).
5) property replacement.
Insurance companies don't look at bikes like we do. When I got hit, the fork
was badly damaged, but the rest of the frame otherwise looks fine (still
pending Ilya's inspection). To a cyclist, just the fork needs to get replaced, a
crank, or a wheel, or whatever. Insurance companies aren’t cyclists and they look
at bikes holistically. If the fork is broken, the whole bike is broken.
Pictures of damage and a receipt from a bike shop stating the bike needs to be
replaced and quoting the replacement value / new value will be required.
6) damages, Scars
and broken bones get more money. Basically anything that is permanent and/or
has a significant amount of recovery time is meaningful. Days off from work
matter. Don't rush back. Listen to the doctors. If you are on the job when you
get hit, you can also qualify for worker's comp (maybe Jim Nelson can opine).
Take all of this as a grain of salt and do your own diligence.
Obviously I am not an attorney. Just takeaways from the one meeting I had with
Phil.

Right - I was told by Adam White (another local bike accident attorney) that Worker's Comp supersedes all other claims. When I was hit recently I was on my way from my office to a client meeting so it applied. All medical bills were forwarded directly to the WC claims administrator.If you are on the job when you get hit, you can also qualify for worker's comp (maybe Jim Nelson can opine).
